Benefits Overview
DEATH PRINCIPLE
During the term of the policy, we will pay this benefit if we receive proof of claim showing that the insured has died. the amount of the death benefit is the amount of life insurance shown on the certificate list, plus any life insuranceof the optional front driver, as well as the whole life insurance. insurance premium paid after the month of death of the insured. , plus anyapplicable interest less premiums remaining before the Covered Person’s death and any accelerated benefits we paidon behalf of the Covered Employee.BASIC PRINCIPLE FOR ACCIDENTAL DEATH, LOSS OF SIGHT AND DISABILITYWe pay the basic benefit for accidental death, loss of sight or sight and dismemberment, if the insured has one of the following as a result of the accidentin case of injury, proof is valid:
• Loss of life or
• Loss of both hands loss or
• loss of both legs or
• loss of one arm and leg or
• loss of vision in both eyes or
• loss of vision in one arm and one eye or
• loss of vision in one leg and one eye.For this benefit, we pay the beneficiary 10% of the life insurance amount as per the life insurance table.Accidental loss of A) both arms, B) both legs, C) vision in both eyes, D) one arm and one leg, E) one arm and one eye,or F) one leg and vision We pay 5 % for the other eye of the life insurance amount according to the obvious table. The damagemust occur within 180 days of the accident.
ACCIDENTAL DEATH, LOSS OF VISION AND INVESTMENT PRINCIPLE
Accidental death benefit is equal to the death benefit provided in the basic plan.We will pay 100% of the accident benefit shown in the certificate table if an employee or spouse accidentallydies. This benefit is paid in addition to other benefits. OrWe pay 50% of the accident compensation if you accidentally lose A) both arms, B) both legs, C) vision in both eyes, D) one armand one leg, E) one arm and vision in one eye, or F) one leg and vision in one eye. OrWe will pay 125% of the benefit for death caused by a motor vehicle or ordinary driver provided the insuredwears a seat belt and:
• drives or is driven by a motor vehicle or
• is a passenger on a common locomotive.This benefit is available to the employee and spouse only.* The rider is included in a fixed life plan. Accidental Death, Loss of Vision and Dismemberment Benefit Rider offers benefits in addition to the Accidental Death, Loss of Vision and Dismemberment benefit on the certificate.
FULL TERMINATION PAYMENT (EMPLOYEE ONLY, 18-60 YEARS)
We waive premiums if an accident or illness before the insured’s 60th birthday results in total incapacity for work. The insurance payment is waived after six (6) consecutive months of total incapacity for work.
ACCELERATED BIRTH BENEFIT
We will pay this benefit when we receive proof of loss showing that the insured has an incurable disease that causes death withinsix months. This benefit is 50% of the life insurance amount stated in the evidence list. The doctor treating the protected person must confirm that the protected person has an incurable disease.
LIMITATIONS AND EXCLUSIONS
If the coverage described in this summary replaces existingcoverage, please note that it may be useful tomaintain an individual warranty renewal policy.
• If the insured, whether sane or insane, dies by suicidewithin two years from the date of the certificate, our liability for wrongful death islimited to the premiums paid.
• If the insured’s age is incorrectly indicated and if thepremium is based on age, the insurance premiums will be adjusted according tothe actual age of the insured.
• If age is used to determine eligibility or sum assuredand the age is incorrectly stated, the coverage, the benefit amounts (or both) for which the insured personis insured will be adjusted according to the actual age ofinsured. Such an incorrect age statement does not continueotherwise valid insurance or terminateotherwise valid insurance.
• If, after the death of the insured person, it turns out thatthe age of the insured person was incorrectly stated, the sum insured will bethe amount for which the insurance premiums would have been purchased at the correcttime. n age
• If the owner does not notify any employeeof the end of the insurance cover during the validity of the general group contract,our responsibility is limited to retroactive reimbursement of the insuranceuntil the day of the insurance. should be valid. is over , less theclaims paid during that period. • All beneficiary changes must be made to us in writing. The changewill take effect from the date of signature.The limitations and exclusions below apply tothe following three benefits: accidental death, loss of sight anddispersion benefits; accidental death, loss of sight anddissolution benefit the rider; andfull disability premium benefit disability.LIMITATIONS AND EXCLUSIONSAccidental death, loss of sight and severance benefits ortotal disability benefits are not payable or availableif the death or loss:
• was caused directly or indirectly, in whole or in part by suicide, orby attempted suicide, whether of sound mind or madness, or of any intentionalself-injury; or
• resulted from an attempt or crime orresisting or escaping arrest; or
• it was caused by or occurred in rebellion orparticipation in rebellion; or
• As a result of voluntarily consuming, inhaling or inhaling poison,noxious gas or vapor; or
• Who intentionally caused someone (if the protected person is aninnocent bystander who has no connection to the dispute, he isprotected). or
• occurred during travel, flight or departure from an aircraftunless the covered person was transported as a paying passengeron a scheduled flight (this exceptiondoes not apply to airline employees flying for work or travel purposes.for pleasure or business travel and back.); or
• Caused by an illness, disease or bacterial infection (ifthe infection is caused by an injury, it is covered).In addition to the exclusions listed above, the following limitationsalso apply to accidental death, loss of sight and dismemberment.Basic Benefit and Rider:
• Loss must occur within 180 days of the accident.
• This benefit ends when this benefit is paid out.
• Substance abuse* (This does not exclude damage caused by the use of drugsprescribed by a doctor and usedaccording to the instructions.);
• War or act of war, whether declared or undeclared;*
• service in the armed forces orauxiliary units of any country or organization;*
• drunkenness;* or
• competition in self-defense – vehicle on racetrack, public road orother place.*If there is a loss covered by this benefit in two or more accidents,we will pay more than 100% of the accident benefit shown on the certificatethe schedule for all such losses. This does not apply to theseat belt benefit.**These exclusions only apply to accidental death, loss of sight andaccident benefits.In addition to the exclusions listed above, the followinglimitations and exclusions apply for total disabilitywaiver of premium:Payment is not taken if total disability:
• Neuroses, psychoneurosis, psychopathy, psychosis ormental and emotional illness or disorder without a demonstrableorganic cause (this exception does not apply to Alzheimer’s disease,Parkinson’s disease or senile dementia).
• Consequences of drug addiction (this exception does not applyto a disease caused by the employee’s use of medicineprescribed by a doctor and taken according to the doctor’s instructions).The insurance payment is waived only if theamed employee appearing on the evidence list suffers from total work disability.Employees insurance does not continue after the employeeturns 65.Damage caused by a previous illness is not compensated, if the damagebegins 12 months after the insured’s entry into force. the insurance of n. TERMS YOU NEED TO KNOW
Accident (Accidental)
means a sudden, unexpected, violent and externalevent that causes bodily injury to a covered person.Common Carrier means a carrier operated by a government regulated or government maintained company; and transport people for a fee.A child is a person who is primarily dependent and lives with the insured in a permanent parent-child relationship, as well as a natural or adopted child of the insured or spouse;
A child placed
for adoption by the insured;or stepson of the insured. A child does not include a person who does not meetthe definition of a child above; a child living outside the United States (if not living with the insured); or a child in active military service for more than 30days.
A covered person
is an eligible employee or dependent whois covered by the plan. Persons entitled to insurance are shown in theschedule.
Eligible dependents
are the spouse, children andchildren of the eligible employee. If the child is covered by the plan,the child’s eligibility does not end if the child is and remains unmarried; incapable of an independent working life due to mental disability or physical disability; and are primarily dependent on an employee or spouse. However, in no event shall the eligibility or coverage of any child continue beyond the date the employee’s coverage ends. The employee mustprovide us with proof of a physical or mental disability within 31 days of the child’s eligibility ending. After thatwe can ask for proof, but not more often than once a year.
An approved employee
is a person who actively works forthe main policy of the group; and meets the enrollment eligibility andwaiting period conditions shown on the application form.
Disease means an illness or disease of a Covered Person.Injury means bodily injury directly caused by an accident and independent of other causes.
Insured means an insured employee who is covered by the plan.
Losing a leg means the complete and irreversible loss of use of the leg.
Hand loss means the complete and irreversible loss of at least four fingerson one hand.
Vision loss means the clinically proven irreversible loss of vision in both eyes as a result of disease or injury.
Corrected visual acuity mustbe less than 20/200; or visual field limitation to 20 degrees or less in both eyes. There must be clear evidence that the blindness was due to the injuryand that the condition has continued without interruptionfor at least six consecutive months after diagnosis. Compensation is not paidif the doctor believes that the operation, device or implant can leadto partial or complete restoration of vision. The diagnosis must be madethrough a physical examination by an ophthalmologist; and after the effective date of the policy.
Motor vehicle means a vehicle authorized to drive on public roads.Physician means a physician or other person recognized by law orregulation in the country where the services are provided as a physician.p
erson must be licensed and practicing in the United States Physician does not include you; a person related to you by blood or marriage; or a physician or other person practicingoutside the United States.A pre-existing condition is a condition causing total incapacity for work thathas been treated by a physician or for which a physician has prescribed treatmentfor an employee within 12 months prior to the employee’seffective date of coverage. .
Seat belt means a safety deviceinstalled by a manufacturer or dealer on a general carrier or motor vehicle, consisting of a strap or harness,designed to restrain an occupant during an accident and reduce injury.
Disease means an illness or disease that causes a loss covered by the plan.Illness includes pregnancy and pregnancy complications.Spouse – a person who is recognized as the husband or wife of the insured under the laws of the country of residence of the insured, or a person whose country of residence the insurer has recognized as the domestic partner of the insured;
party to the civil union ended with the insured;reciprocal beneficiary of the insurer; or anyone to whom we are required by the laws or regulations of that stateto provideplan coverage, or persons who may be covered by the plan by marriage pursuant to a written agreement between the Company andpolicyholder.
Terminal Illness means an Illness which, with reasonablemedical certainty, will cause the death of the person covered by the Planwithin six months from the date the claim is signed by the attending physician.The attending physician must certify that the eligible employee or dependentis suffering from an incurable disease that begins during theplan period.
Total disability (total disability) meansthat, during the first 12 months of disability, the covered employee is unable to perform the essentialand material duties of normal occupation; does not work in any otherprofession; and under the care of a pediatrician.After 12 months of total incapacity for work, total disability means that the insuredis unable to perform the duties of gainful employment for which he has the education, training or experience; and under the care of a disabled physician. We do not need medical care unlessit is no longer neededfor the medical treatment of a disease that causes total disability..
